I greatly appreciate your interest in having me do a tattoo for you, but before setting up the appointment I would appreciate you reading my Terms of Service before submitting a form. This helps me to ensure I am able to answer and hopefully book as many people as I can as quickly as I am able to. Once you have done so fill out all areas of this form (including attaching a reference image) and I will respond to you about your request ASAP! Please be patient if it takes me a up to a week to respond as I do get a lot of messages that I am trying to answer along with taking care of existing clients. I work Tuesday through Saturday so weekend emails may not be responded to until the following Tuesday. If you need to get ahold of me urgently you can text or call me at 1(585)648-8935. Lastly, PLEASE MAKE SURE YOUR CONTACT INFORMATION IS CORRECT! If its not correct I can't reply!
